  • RN, CASE MANAGER
  • Coordinates discharge planning, implements utilization management functions and case management on high risk medically complex patients across the continuum of care. Determines appropriate level of care according to interqual and milliman criteria. Follows TJC and CMS standards as it applies to discharge planning and utilization review.
  • Requirements:
  • 2 years clinical nurse experience required and 1 year utilization review/case management preferred. BSN required.
